17. Adding Forms
Forms allow service providers to collect signed documents, like waivers, before bookings.
17.1 Before you begin
- Sign in with a
practice-adminaccount.
17.2 Create a form template
- Open Forms from the left sidebar under My Service Provider.
- Select New form.
- Enter a name and the template content.
- The editor supports Markdown for headings, lists, and links.
- Set Last updated using the date selector.
- Save the form.
17.3 Attach a form to an event
- Open the event you want to update.
- Choose the form template in the event settings.
- Save the event.
17.4 Tips
- Keep language clear and client-friendly.
- Update the Last updated date any time terms change.
