17. Adding Forms

Forms allow service providers to collect signed documents, like waivers, before bookings.

17.1 Before you begin

  • Sign in with a practice-admin account.

17.2 Create a form template

  1. Open Forms from the left sidebar under My Service Provider.
  2. Select New form.
  3. Enter a name and the template content.
  4. The editor supports Markdown for headings, lists, and links.
  5. Set Last updated using the date selector.
  6. Save the form.

17.3 Attach a form to an event

  1. Open the event you want to update.
  2. Choose the form template in the event settings.
  3. Save the event.

17.4 Tips

  • Keep language clear and client-friendly.
  • Update the Last updated date any time terms change.
